Enrollment Procedures

When you select the Center for Family Medicine for your health care, we ask that you complete enrollment forms even before you need to make a visit.  This enables us to make an appointment for you quickly when necessary, and also assures that your doctor will have your basic medical information.  Please call our office (216) 476-7088) for an enrollment package, or stop by our office to pick one up.  

Once the enrollment forms are completed and returned to us, the Center for Family Medicine will recommend one of our physicans to be your new personal family doctor.  All routine visits will be made with your personal physician.  For urgent, same-day visits, you might see one of our other doctors. 

 

Advance Appointments

Please call to schedule appointments as far in advance as possible.  This will help us in scheduling a time that is most convenient for you.  It is also important to tell the receptionist all of the medical concerns that you wish to discuss with the doctor.  This will ensure that you have enough time to discuss your concerns with your doctor. 

 

Same-Day Appointments

We reserve a number of office appointments each day for urgent conditions that must be treated immediately.  If your medical condition requires an urgent visit, please call as early in the day as possible.  For an urgent or same-day appointment, you may see a physician other than your personal doctor if he or she is not available. 

Cancellations

If you must cancel your appointment, please call as soon as possible in advance.  This will allow us to give your appointment to another patient.
